We are pleased to share that Curve Lake First Nation will be hosting COVID-19 Vaccination
clinics at the community centre on March 3rd and March 6th.
Those who have completed an expression of interest form will be contacted by the Health and
Family Services Team to schedule their appointment times.
If you have not yet submitted an expression of interest form and are interested in the vaccination
it is not too late. The form is available online and also in paper form at the Health Centre. If you
prefer you can call the Health Centre at 705-657-2557 to complete the form over the phone.
If you have completed an expression of interest form and have not been scheduled for an
appointment by March 1st at 12 noon please contact the Health Centre to ensure your space
We will be receiving the Pfizer vaccine and it is imperative you speak to your health care
professional if you have any questions or concerns about whether the vaccine is right for you.
For those at higher risk during the vaccination process, arrangements will be made to schedule
you an appointment for vaccination at PRHC (these appointments may be on different dates than
our community vaccination).
You should not get the vaccine if you have had a severe allergic reaction to any ingredient in the

The Pfizer-BioNTech COVID-19 Vaccine includes the following ingredients: mRNA, lipids ((4-
hydroxybutyl)azanediyl)bis(hexane-6,1-diyl)bis(2-hexyldecanoate), 2 [(polyethylene glycol)-
2000]-N,N-ditetradecylacetamide, 1,2-Distearoyl-sn-glycero-3-phosphocholine, and
cholesterol), potassium chloride, monobasic potassium phosphate, sodium chloride, dibasic
sodium phosphate dihydrate, and sucrose.
