Dental Benefit Information

The Orthodontic Policy has been updated to include a Modified LD Index within the criteria for orthodontic coverage. This policy has enhanced the criteria for review by considering pain and discomfort associated with malocclusion.
Denied claims between March 24, 2016 – July 31, 2018 can be resubmitted for review considering policy change.
